Latrobe Community Services to offer transitional care

Uniting AgeWell’s Latrobe Community Services set to help people recover and regain their independence

Uniting AgeWell is delighted to announce its selection by the Tasmanian Health Service as a preferred provider of 19 Transition Care Community Packages for North West Tasmania.

Transition Care Packages (TCP) offer short-term, goal-focused therapy and case management for older people who need more time to recover after a hospital stay.

The packages will be delivered by Uniting AgeWell’s Latrobe Community Services team based at Strathdevon, lead by Manager Integrated Services, Vicki Pollock and Coordinator, Peter Cullen. 

According to Vicki, the benefits of TCP are numerous.

“For some older people, returning home after a hospital stay is not always possible; sometimes it’s necessary to enter a respite facility to aid recovery or to make a rushed decision to move into residential care.  Knowing people prefer to be in their own home while they get back on their feet or make complex and important decisions about their future, is where a TCP can help,” Vicky said.

“Being able to offer these TCPs will give people in North West Tasmania the opportunity to be supported by UA staff to stay at home and recover over a 12 week process.  Clients will be actively involved in their case management, care and therapy and will set the goals for their recovery that we will all work towards.

“We feel very honoured that the Tasmanian Health Service has granted us these transition packages and we believe that we have the staff and services to provide older people with the choice to live where they want, and in the way they want."

The contract for TCP services will commence on 1 January 2017.  When fully operational, Uniting AgeWell will support up to 76 clients each year. To contact the program, call (03) 6426 2844.

UA’s Latrobe Community at Strathdevon already offers independent living units, home care packages, social support, residential care and provides the meals for Meals on Wheels on the north west coast.
