Tasmanian Health Services has extended its Transition Care Program (TCP) – Community Packages contract with Uniting AgeWell by another two years.
Executive Director of Operations Eric Daniels last month wrote to General Manager Strategy and Business Development Fonda Voukelatos informing him the contract for TCP in North West Tasmania had been extended until 31 January, 2021.
Home Care Programs, Tasmania, Regional Manager Fiona Onslow-Agnew said it was “kudos” to the team that took over the contract from another provider just two years ago.
“We have a really good working relationship with the hospital and they are happy we’re able to get people out of hospital smoothly, with less issues,” she said.
“The feedback has been very positive from the clients.”
Through the agreement, Uniting AgeWell prepares a discharge plan for each client and delivers a tailored 12-week program to support them to remain well or regain their independence at home.
The North West Community Services Transition Care Program Team was last month awarded Uniting AgeWell’s 2018 Team Innovation Award, which recognises the outstanding effort of a group of UA staff working together to develop and implement new ways of providing a service.
The team was applauded creating innovative programs to support people to reintegrate with their communities and remain independent at home.
They have established a number of social groups, with up to 100 people attending some events. The positive outcomes of the service are a testament to the great innovation of this team.
